First Visit

Prior to your visit, please complete the New Patient Information Form and return to us.^★

On the day of your appointment, please bring with you:

• Referral letter from GP, family physician or other doctor
• Medicare card, and if you have one, your DVA card or Pension Card
• Your private hospital health insurance information
• Provider (e.g. SKG or PRC) of X-rays, MRI scans, CT scans and ultrasound scans
• Any other relevant information
